19 Mandatory conditions where licence authorises supply of alcohol
(1) Where a premise licence authorises the supply of alcohol, the licence must include the following conditions.
(2) The first condition is that no supply of alcohol may be made under the premises licence -
(a) at a time when there is no designated premises supervisor in respect of the premises licence, or
(b) at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
(3) The second condition is that every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
Annex 2 - Conditions consistent with the Operating Schedule
General
There will be clearly displayed posters at the entrance of the premises and throughout the premises to state that we employ a Challenge 21 policy to communicate that where a person appears to be under 21 years of age, they will be asked to provide an adequate proof of age card accredited under the Proof of Age Standards Scheme (PASS) or if a proof of age card is not available, a photo driving licence, passport or accredited student card.
There shall be in place for the premises a policy to prevent the sale or supply of alcohol to persons under 18 years of age. That policy shall require any person who appears to be under the age of 21 years to produce a recognised proof of age card accredited under the Proof of Age Standards Scheme (PASS) or if a proof of age card is not available, a photo driving licence, passport or accredited student card.
A notice or notices shall be displayed in or at the entrance to the premises where they can be clearly seen and read and shall indicate that it is unlawful for persons under 18 to purchase alcohol or for any person to purchase alcohol on behalf of a person under 18 years of age.
There shall be displayed at thel entrance to the premises a sign, which is clearly visible to members of the public visiting those premises containing details of the trading hours under the premises licence, the licensable activities permitted on those premises and the maximum capacity level set for the premises.
The prevention of crime and disorder
The Designated Premises Supervisor, when present, and all members of staff shall ensure that all lawful instructions and/or directions given by the Police are complied with.
No drink shall be removed from the premises in an unsealed container.
A CCTV camera shall be installed at the locations marked, ‘CCTV CAMERA’ on the plan of the premises.
Where CCTV equipment is fitted, it shall be maintained in good working order in accordance with the manufacturers instructions.
The images recorded by the CCTV system shallbe retained in unedited form for a period of not less than 31 days.
The maximum number of persons allowed in these premises shall be 30 people. For members of the public, this covers the main sales area only and for members of staff, the main sales area, the office/store room, the rear store area and store room, sink area and bathroom.

Public Safety
Where disabled persons are present on the premises there shallbe in place arrangements for their safe evacuation in case of an emergency or other reason. Details of those arrangements shall be recorded in writing and all staff employed on the premises shall be aware of those arrangements.
All disabled persons on the premises shall be made aware of the evacuation arrangements by way of appropriate notices displayed in easily visible locations.
All exits and escape routes to those exits (including external escape routes) from the premises shall be provided with non slip even surfaces and shall be maintained in good repair, correct working order and kept free from all obstructions when the public are on the premises. External escape routes shall also be kept free from ice and snow.
Any outbreak of fire of any kind shall be reported to the Fire Service immediately and a written record shall be made in a logbook kept for that purpose.
There shall be maintained on the premises at all times an adequate and appropriate supply of first aid equipment and materials.
When the public, members or guests are present, the lighting levels in all parts of the premises shall be capable of providing sufficient illumination for the publuc, members or guests to leave the premises safely.
The prevention of public nuisance
The licensed premises or the parts of the licensed premises edged in red on the supplied plan shall be open to the public and their guest between the hours of 8:00 to 20:00 Monday to Saturday and 10:00 to 20:00 on Sundays.
The protection of children from harm
There shall be in place for the premises a policy to prevent the sale or supply of alcohol to persons under 18 years of age. That policy shall require any person who appears to be under the age of 21 years to produce a recognised proof of age card accredited under the Proof of Age Standards Scheme (PASS) or if a proof of age card is not available, a photo driving licence, passport or accredited student card.

All staff will be adequately trained to ensure compliance with the law in relation to sale/consumption of alcohol by persons under 18 years of age, including prevention of adults buying alcohol for children

To support the Challenge 21 policy, a refusal record book will be used when any sales of alcohol are refused because a person could not provide adequate proof of age identification.
