Annex 1 - Mandatory conditions
Mandatory conditions where licence authorises supply of alcohol
(1) Where a premise licence authorises the supply of alcohol, the licence must include the following conditions.
(2) The first condition is that no supply of alcohol may be made under the premises licence -
(a) at a time when there is no designated premises supervisor in respect of the premises licence, or
(b) at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
(3) The second condition is that every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
ADDITIONAL MANDATORY CONDITIONS
1. (1) The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol
(2) The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark
Annex 2 - Conditions consistent with the Operating Schedule
General - All Four Licensing Objectives (b,c,d,e)
There will be clearly displayed posters at the entrance of the premises and throughout the premises to state that we employ a Challenge 25 policy to communicate that where a person appears to be under 25 years of age, they will be asked to provide an adequate proof of age card accredited under the Proof Of Age Standards Scheme (PASS) or it not available, a photo driving licence, passport or accredited student card.
There shall be in place for the premises a written policy to prevent the sale or supply of alcohol to persons under 18 years of age. That policy shall require any person who appears to be under the age of 18 years to produce a recognised proof of age card accredited under the Proof of Age Standards Scheme (PASS) or if a proof of age card is not available a photo driving licence, passport or accredited student card.
A notice shall be displayed in the premises where it can be clearly seen and read and shall indicate that it is unlawful for persons under 18 to purchase alcohol or for any person to purchase alcohol on behalf of a person under 18 years of age.
There shall be displayed at the entrance to the premises a sign, which is clearly visible to members of the public visiting those premises containing details of the trading hours under the premises licence, the licensable activities permitted on those premises and any maximum occupancy level set for the premises.
The Prevention Of Crime & Disorder
The Designated Premises Supervisor, when present, and all members of staff shall ensure that all lawful instructions and /or directions given by the Police are complied with.
No drink shall be removed from the premises in an unsealed container.
A CCTV camera shall be installed at the locations marked, "CCTV CAMERA" on the plan of the premises.
The maximum number of persons allowed in these premises shall be 30 people. For members of the public, this covers the main sales area only and for members of staff, this would cover the main sales area, the office/store room, the rear store area and store room, sink area and bathroom.

Public Safety
When disabled persons are present on the premises there shall be in place arrangements for their safe evacuation in the case of an emergency or other reason. Details of those arrangements shall be recorded in writing and all staff employed on the premises shall be aware of those arrangements.
All disabled persons on the premises shall be made aware of the evacuation arrangements by way of appropriate fire exit notices displayed in easily visible locations.
All exits and escape routes to those exits from the premises shall be provided with good even surfaces and shall be maintained in good repair, and kept free from all obstructions when the public are on the premises. External escape routes shall be kept free from ice and snow.
Any outbreak of fire of any kind shall be reported to the Fire Service immediately and a written record shall be made in a logbook kept for that purpose.
There shall be maintained on the premises at all times an adequate and appropriate supply of first aid equipment and materials.
When members of the public are present, the lighting levels in all parts of the premises shall be capable of providing sufficient illumination for people to leave the premises safety.
The Prevention Of Public Nuisance
The licensed premises or the parts of the licensed premises edged in red on the attached plan of the premises may open to the public between the hours of 08:00 to 23:00 Monday to Saturday and 10:00 to 21:00 on Sundays.
The Protection Of Children From Harm
There shall be in place for the premises a written policy to prevent the sale or supply of alcohol to persons under 18 years of age. That policy shall require any person who appears to be under the age of 25 years to produce a recognised proof of age card accredited under PASS or if not available, a photo driving licence, passport or accredited student card.
A notice shall be displayed in the premises where it can be clearly seen and read and shall indicate that it is unlawful for persons under 18 to purchase alcohol or for any person to purchase alcohol on behalf of a person under 18 years of age.
All staff will be adequately trained to ensure compliance with the law in relation to the sale / consumption of alcohol by persons under 18 years of age, including the prevention of adults purchasing alcohol for children.
There will be clearly displayed posters at the entrance of the premises and throughout the premises to state that we employ a Challenge 25 policy to communicate that where a person appears to be under 25 years, they will be asked to provide an adequate proof of age card accredited under the Proof Of Age Standards Scheme (PASS) or if not available, a photo driving licence, passport or accredited student card.
To support the Challenge 25 Policy, a refusal record book will be used when any sales of alcohol are refused because a person could not provide adequate proof of age identification.
